For the past couple of weeks, my Bluetooth mouse and headphones have been disconnecting at random, regardless of what the PC is doing. Sometimes they reconnect quickly, but other times they stay disconnected. I'm using a desktop tower with built-in Bluetooth, specifically a Legion T3. I've already tried forgetting and re-pairing the devices, disabling power management for the Bluetooth adapter, updating the Bluetooth and device drivers, updating the BIOS, reinstalling the Bluetooth drivers, and a few less scientific troubleshooting methods. Nothing has made the connection reliable. What else could be causing this?

Make sure the Wi‑Fi/Bluetooth antennas are firmly connected to the coaxial ports on the motherboard. Missing or loose antennas are a very common cause of weak or unstable Bluetooth connections on desktop systems.
Since this is a tower with built-in Bluetooth rather than a USB dongle, the exact motherboard or wireless card could help narrow it down. Desktop Bluetooth can be sensitive to antenna placement, USB 3 interference, and problems with the internal wireless adapter. Checking the hardware model and testing with a separate Bluetooth adapter would help determine whether the built-in radio is failing.
The antennas are connected, and I also tested with them removed; the disconnects still happened, so that doesn’t seem to be the cause.
