I replaced a working RTX 3050 with a new GIGABYTE RTX 5060 Ti 8GB in a system with an ASUS ROG Strix Z690-A Gaming WiFi D4 motherboard, Intel i7-12700KF, Corsair RM1000x 1000W PSU, and Windows 11. The 5060 Ti appears in Device Manager with NVIDIA driver 576.52, but Windows reports: "Windows has stopped this device because it has reported problems. (Code 43)."
The display is limited to 1024x768, the monitor appears as a Generic PnP Monitor instead of an LG UltraGear, the NVIDIA App says the drivers are not installed, and the NVIDIA Control Panel option is missing.
I already used DDU in Safe Mode and reinstalled drivers 576.52 and 576.02, verified that the correct PCIe power cable is connected, confirmed the NVIDIA Display Container LS service is running, and connected the monitor directly to the GPU by DisplayPort. I also reseated the card and tested it in the second PCIe slot, but Code 43 remained. The motherboard BIOS was updated recently as well. Does this sound like a compatibility or hardware problem, or is there anything else worth trying before requesting an RMA?

Try reseating the card and, if possible, testing it in another full-length PCIe slot. You can also test another known-good GPU in the same system. If the other GPU works normally while the 5060 Ti still shows Code 43, the card itself is probably faulty.
Code 43 means Windows can see the card and install a driver, but the driver cannot properly communicate with the hardware. That explains the fallback resolution, missing NVIDIA settings, and the NVIDIA App claiming the driver is unavailable. Since DDU did not help, a defective card is a strong possibility.

I reseated it and tested the second PCIe slot, but the exact same Code 43 error appeared.
