For the past two months, I've been getting login requests on my phone via the MS Authenticator app for my main Outlook email account. This involves selecting one of three number choices to approve the request. Sometimes I get several requests in a single day and sometimes none for days. Since then, I've changed my password twice, yet these requests keep coming in, even within an hour after changing my password. I scanned my PC for viruses and it's clean, so I'm not sure what's happening. I've also denied numerous attempts coming from the US, Netherlands, and mainly from Germany, which is where I am located.
I'm quite tech-savvy and notice that none of these denied login attempts show up in my Activity page, along with my recent password changes. The only one recorded is from December 2025, which doesn't help my current situation. I'm starting to think there's an issue with the MS Authenticator app itself rather than someone actively trying to hack my account. I've considered disabling the "Send sign-in notification" option as I might switch away from the MS Authenticator altogether. Additionally, I've been using a VPN since November, and I wonder if that could be affecting things too.
Is there a straightforward way to reinstall the Authenticator app without losing all my accounts? Maybe clearing the cache would help?

You might want to check if your account has any linked devices that you're not aware of. Sometimes apps or old devices can trigger requests. I noticed the same thing happening with mine and had to go through old device settings and disconnect some of them. As for reinstalling the Authenticator, you could back it up first if you have that option; otherwise, a cache clear could keep your accounts intact.
It sounds like you're in a frustrating situation! Since you're tech-savvy, I suggest checking your account security settings and enabling two-factor authentication if you haven't done so already. It adds an extra layer of security. Regarding the Authenticator app, it might help to clear the app cache first—try that before full reinstallation. If those login requests continue, consider reviewing any apps connected to your account that might be causing these calls, like your VPN. Sometimes, unusual behavior can stem from settings there.
I hear you on the confusion with the sign-in requests! Same thing happened to me with attempts that weren't reflected in my activity page. I had issues with my VPN, too—requests kept showing up from countries where I wasn't logged in. Maybe switching your VPN off for a bit might help clarify if that's the problem. Don't hesitate to reach out to Microsoft support, they've been helpful for me in the past.
