I see two options: I can either make a reservation for about $58 at the store or order an express replacement for $99. Since there's an Apple Store close to me, I think going in makes sense. I understand that if they can fix it on the spot, I'll pay the lower fee, but if they need to replace the phone, I'll end up paying the $99 regardless. I have a few questions: 1) Should I still make a reservation with my phone's level of damage? 2) If I go for the $58 reservation and they decide it needs replacement, will I end up paying $58 plus $99, or just $99? 3) If I choose express replacement, could the cost ever go over $99, or is that the cap under AppleCare+?

Both options are solid! If you're near the store, I’d say skip the express replacement to avoid a hold on your card. Just pay the repair fee, and if they can't repair it, you'll only be charged $99 tops. So, don’t stress too much about it!
