How can I troubleshoot a painful high-frequency noise from my new PC?

I built a new PC around an AMD Ryzen 7 9800X3D, but it produces a high-frequency sound that I can barely hear normally yet seems to cause ear fullness and physical pain. I cannot use the computer for more than about 15 minutes before I need to turn it off and rest. My older PCs never caused this, although I have had a similar reaction to certain audiophile speaker and amplifier setups in the past.

I have already replaced nearly everything except the CPU: two 1000 W power supplies from different manufacturers, two X870E motherboards, three cases, two air coolers, two memory kits, and several SSDs. I also tested the computer at a friend's house to rule out my home's electrical system. I disconnected the case fans, removed the RTX 5080, and am currently using the integrated graphics. The noise remains even when the computer is sleeping, although enabling ErP may have changed it slightly.

I have tried disabling C-states, changing CPU load-line settings, and switching or disabling EXPO. The memory settings seem to affect the sound somewhat, but leaving EXPO disabled is not a practical solution. I am considering replacing the 9800X3D with another CPU, such as a 9850X3D or 9950X3D, but I would rather try a more likely fix first. Could the CPU be interacting with the motherboard VRM or power delivery in a way that creates ultrasonic coil whine? Would undervolting or using a different, higher-end motherboard be worth trying?

One practical way to isolate the source is to put the computer in another room and run long display and USB cables, assuming you can do that temporarily. I have encountered systems and monitors that produced irritating frequencies even when other people could not hear them, and moving the tower away solved the problem. If distance changes the symptoms, test the monitor and peripherals separately as well as the PC. Since this is causing physical pain, avoid trying to tolerate it for long periods and consider getting medical advice about the ear symptoms too.

Moving the computer is difficult because it has to stay in the same room, but I will try to identify whether the monitor, cables, or a particular motherboard and CPU combination is responsible rather than assuming the processor is the source.

This sounds more like coil whine or another high-frequency electrical resonance than ordinary fan noise. The source could be a motherboard VRM, the power supply, a monitor, or even a cable or connected peripheral. Since the behavior changed with EXPO, memory settings may be changing the SoC or VRM load and affecting the resonance. A different CPU could change the electrical conditions, but it seems less likely than finding a motherboard and power-delivery combination that does not resonate.

Undervolting or reducing the CPU's power limits is worth trying before buying another processor. It costs nothing and may change the electrical load enough to alter a VRM coil-whine resonance. I would focus on the motherboard's CPU and SoC power stages first, since the CPU itself has no obvious moving parts that would normally generate this kind of sound. If possible, make the change from another device or use a short test session, since you should not keep exposing yourself to a sound that causes pain.

I have never undervolted before, and I can only stay near the machine for roughly 15 minutes. Is this likely enough of a possibility to justify trying it before replacing the CPU?
