Is my iPad being hacked?

I'm really worried because I think someone might be accessing my iPad's apps, microphone, and camera. Here's what happened: I have an iPad Mini with an A17 Pro chip. Earlier today, I accidentally clicked on a malicious website and saw a popup asking to allow access to my microphone and camera. I tried to deny it, but the screen froze. After about 15 seconds, Safari crashed, and when I reopened it, all my tabs and history were gone.

Concerned, I searched about malware and turned on the app privacy report in my settings. Later, I noticed a green circle at the top right of my screen, which I read means someone is using the camera. It disappeared, but when I checked the app privacy report, I saw apps like Messages, Photos, Mail, and Books appearing in the history that I haven't accessed. It keeps updating with times like "entered 9 mins ago" and has a deleted app called "ShortcutsAction" that I didn't download myself. Am I being hacked? Is my information at risk? Will factory resetting my device fix this issue?

It does sound like there's a good chance your iPad might be compromised. First, make sure to back up all your important videos and photos. After that, perform a factory reset to wipe everything clean. Also, it’s crucial to change passwords for any accounts accessed on that iPad, like banking or social media accounts, since it’s likely they could be compromised too.

Just to add to that: if you're using the same Apple account across multiple devices, there's a risk that the issue could affect those as well. Be sure to monitor your other devices for suspicious activity after you reset your iPad.
