My wife doesn't remember intentionally signing up for them, so we're trying to figure out whether they came from a new-device setup, an in-app offer, or a free trial that automatically converted to paid billing.
Is there a way to see exactly when and how each subscription was started, such as which app or promotion initiated it? Also, has anyone successfully received a refund for unexpected subscription charges? We've already canceled everything so the subscriptions won't renew. This has made me even more cautious about free trials, since they can quietly turn into paid plans if they aren't canceled in time.

It may have been an Apple One-style bundle or several offers accepted during the setup of a new phone. Check the App Store purchase history under the account profile and look at the subscription details. The history may identify an initial subscription, and you may need to change the filters to include free purchases so you can find the trial that later became paid.
If Apple doesn’t resolve it directly, contacting the card issuer may be another option. In one case, the issuer couldn’t identify the originating app but was able to reverse the charges. Results can vary, and it’s still best to cancel the subscriptions and submit the refund request to Apple first.
Apple’s own apps commonly offer free trials when you first open them, and those trials can automatically become paid subscriptions. You can review more specific purchase and billing information through Apple’s purchase-reporting page, then request a refund for eligible charges there.
It’s also worth checking whether the subscriptions were part of a promotion connected to an Apple Store purchase or a new device. A purchase several months earlier could explain why the paid charges began later, once a promotional trial ended.

That seems likely. She got a new phone a few months earlier, so she may have accepted a three-month trial and only noticed it once billing started. It’s a good reminder to track trial end dates, even when the initial offer is free.
