You might want to check out Zoom Phone along with an Obi adapter for physical fax machines. I've been using it and it's awesome—great pricing and it just works better than other options I've tried.

If you're okay with handling a system by yourself, consider using something like voip.ms with a softphone app like GroundWire. I use it for my own side business and it works well. Alternatively, local VoIP providers can fully manage your system, but expect to pay around $30-$35 per user and the quality can be hit or miss until you've signed a contract.
