I added an iPhone 5c for extra verification codes, but after removing it, my iPhone 6s keeps asking for both my iCloud and Mac passwords. It accepts the first two steps of the login process, but it always says the password is wrong on the third step, even though I know it's correct. I had a similar problem before. It worked for me when I deleted my account from the iCloud settings and then added it back again. Be careful with your encrypted data though—make sure you back up anything important before trying that!
