I had a similar situation where my USB wasn't being read because of the format. Make sure you're using NTFS instead of FAT32 when you create your bootable disk. I ran into a problem where the tool didn't copy all files correctly. Switching to NTFS worked for me on the first try!
If it seems like Windows isn't installing correctly, you could try booting from a Linux distro. Download one that contains GParted, and use it to delete all partitions on your Windows drive. After that, create a new bootable USB with the Windows installer and see if that does the trick!
