Why is ChatGPT Hallucinating After Uploading Documents?

I've been having a frustrating experience with ChatGPT lately. Whenever I upload documents (both word and pdf formats) and ask it to summarize the content, it just goes off on tangents and makes up information that's not even in the document. I primarily use it for creative writing to check grammar, lore, and inconsistencies, but now it's throwing out completely unrelated sci-fi elements. I've tried clearing memory, logging in and out, but it seems to be getting worse instead of better. Can anyone explain what's happening here?

It could depend on how the document is formatted or converted. Using something called OCR might help it better understand your PDF content. If you can edit the Word file after conversion, that might make a difference in accuracy. I've been trying that with some success.

I think it might just be a bug. I tried uploading the same document I used last week, and it completely missed the point this time too! It seems hit or miss these days, but some users mentioned that using Notebook LM has helped them with similar issues.
