How do I read the values from CrystalDiskInfo for my SSD?

I have a 256GB SSD that's showing some signs of aging, like really slow write speeds (sometimes a bit faster, though) and Windows indicating that the usable space is just 237GB. I decided to check its health using CrystalDiskInfo, but it shows that all current and worst values are 100, which confuses me because there are high raw values as well. Honestly, it might be time to back up any important data and consider replacing the drive. How old is it?

It's at least 3 years old, probably more.
