For about a week, Windows 11 has been repeatedly playing the USB unplug/disconnect sound. It happens anywhere from every few seconds to every 20 minutes, sometimes continuously for several minutes. I previously removed a USB hub because I was overloading the available USB connections and devices were actually cutting out. After rearranging everything, the devices appeared stable, but the disconnect sound continued.
My setup includes keyboard and mouse dongles, a microphone, a webcam, six Nintendo DS capture cards, and an external capture card. Two DS capture cards are connected through a USB expansion card, two are connected directly to the rear motherboard ports, one is connected near the webcam, and one is connected to the front-panel port. The capture cards are limited to two per USB port section.
The camera, microphone, capture cards, keyboard, and mouse do not visibly stop working when the sound plays. Event Viewer and Device Manager are difficult to use because the device list refreshes constantly. I have tried disconnecting and reconnecting the devices, changing the USB layout, and restarting the computer. At one point, two DS capture cards stopped appearing in the capture software even though their charging lights came on and Windows played the connection sound. What could be causing the repeated disconnects, and how can I identify the problematic USB device or port?

The hub is worth investigating, especially since it is an Insignia model and the system previously had USB devices cutting out under load. Test the setup without that hub and try the affected devices on different motherboard ports. A device can receive enough power for its charging light while still failing to establish a proper data connection.
Start by disconnecting everything except the keyboard and mouse, then reconnect the other devices one at a time. That should help identify whether a particular capture card, cable, port, or hub is triggering the sound. Since several high-bandwidth devices are connected, a properly powered USB hub may also help prevent power or bandwidth problems.
I reconnected the devices and initially all six capture cards appeared, while the webcam, microphone, and external capture card also worked. After restarting, two DS capture cards were no longer detected even though their lights came on and Windows played the connection sound. I am going to keep testing different layouts and individual devices.
That seems possible because the two affected capture cards now only show signs of charging and are not recognized by the capture software. I rarely move or unplug them, so I will compare them with known-good ports and devices.
