If a project already has comprehensive test cases and strong code coverage, what additional value does a separate test environment provide? I'm trying to understand which kinds of problems a test environment can reveal that automated or planned test cases might miss.

Tests can verify the behavior you expected, but they won’t necessarily catch every issue that appears when the whole system runs together. A separate environment is useful for integration, end-to-end, performance, configuration, deployment, and exploratory testing.
Unit tests mostly prove that individual pieces work in isolation. A test environment lets you check how those pieces interact: the application, database, queues, external services, authentication, networking, and deployment configuration. It can also expose race conditions and other system-level problems that are hard to model in individual test cases.
Coverage numbers don’t necessarily mean realistic coverage. It’s worth testing complete API-to-database flows, queries against substantial and representative data, and communication with real or realistic service endpoints. Testers also tend to try unusual inputs and workflows—invalid dates, strange names, missing values, unexpected sequences—that developers may not think to include in scripted cases.
A shared test environment also provides reproducibility. It gives the team a controlled place to verify that the application behaves as it will after deployment, instead of relying on everyone’s slightly different local setup. That helps distinguish actual product bugs from environment or configuration problems.

For example, a change can pass every local test but time out in QA because the production-like database contains far more data. The query may perform perfectly against a small local dataset and poorly against a much larger one.
