When I first connected my TV, I was offered options to mirror, extend, or otherwise configure the external display. Those choices no longer appear in the display settings, and disconnecting and reconnecting the TV does not bring the menu back. The external display is also limited to 4K at 30 Hz, even though an optical HDMI cable that supports 4K at 120 Hz works correctly in Windows. Could the refresh-rate limitation be related to using an extended desktop instead of mirroring, and how can I access the display mode options again?

The missing options are a little unusual, so a screenshot of the available display settings would help identify what is going on. The 4K 30 Hz limit is not normally caused simply by using an extended desktop; check the selected output mode, graphics driver, HDMI port, and whether the display is being detected with the expected capabilities.
