I recently browsed some NSFW images on a website and realized my iPhone is one update behind. Am I at risk of having been attacked during this browsing session? How likely is it for media files to carry browser attacks?

You're probably worrying too much! Sure, there are some exploits out there, but they’re not common. Even if there was a vulnerability, being on the latest OS wouldn’t necessarily protect you—a lot of exploits are often unknown until they're patched. Overall, your late update isn’t a big concern for the kind of browsing you did.
