I accidentally added money to my Apple Account Balance instead of Apple Cash because the App Store made it confusing. I called support right away and asked for a refund. They said it could take up to 48 hours to process. I'm worried I might run into issues. The support agent said they didn't foresee any problems, but when I asked if she had dealt with this type of situation before, she said no. What do you think will happen with my refund? Will they return the money to the same payment method I used, or is it likely to go into my Apple Cash instead?

As long as the full amount you added remains in your Apple Account Balance, it should get refunded. Usually, refunds hit your original payment method within 24 hours after approval, but the bank will take some time to process that. Just be aware that there’s no instant refund option. Also, for future deposits to Apple Cash, be sure to follow the proper steps in the Wallet app!
There's almost no chance they'll move those funds to your Apple Cash. The refund process usually takes up to 48 hours. If it gets approved, the money goes back to the card used for the purchase, but keep in mind that it can take up to 30 days to reflect in your account. However, it’s typically quicker, often just a few days. After that, you can re-add the cash to your Apple Cash.
This is exactly what I needed to know, thanks!
