Why did a Windows.old folder show up on my PC without formatting?

Hey folks! I've noticed a Windows.old folder has appeared on my computer, but I definitely didn't format my system. Now, I'm also seeing two operating system options when I boot, and the second one doesn't work. Any ideas on what might be going on?

The Windows.old folder is created to let you roll back your system after a major update. Seeing two operating system choices might happen if you have multiple partitions or if the boot manager thinks there’s a second install when there isn’t actually one.
