I've been using my laptop for downloading and watching adult content for years, and I find it hard to believe that my device is completely clean, even though my antivirus (McAfee) says everything is fine with no viruses or malware detected. My concern is, if there happens to be a hidden Trojan or something lurking around, will a full system reset completely wipe it out, or could some malware still stick around after the reset?

Downloading adult content by itself won't give you malware, haha. It’s similar to how STDs work—you can only get malware if you install something and authorize it. So just make sure you're careful about what you're downloading.
If you're planning to do a clean format, make sure you're using Windows media to delete all partitions from your disk during the reinstall. That way, you can be pretty sure you're safe afterward. Also, if you have any extra drives, I recommend cleaning those too for peace of mind!
