User A wants to read User B's inbox, and I can grant User A permission through the 365 admin center by giving them 'Read and manage permissions'. My question is, can User B see that User A has access if they check their Outlook? Just trying to figure out the privacy side of things.

If you grant access and User B checks their mailbox settings, they will see who has delegate access. So that could give them a heads-up if they do some digging!
Generally, User B won't know if User A has been granted access unless User A starts reading unread emails, which would mark them as read. Otherwise, it should be pretty stealthy as long as nothing unusual happens in the mailbox.
Why not give it a shot? Create a couple of temp accounts, give access to those, and see what feedback you get. It’s a good way to confirm how it works in practice!
I’m not a lawyer, but I’d assume that logging would happen when you grant access. Audit logs typically track these changes, and it might raise flags if someone looks too much. Always better to have a paper trail just in case.
