How can I tell if I’ve been hacked and what should I do?

I think my accounts have been compromised since I noticed someone logging in from a different country. Even after changing passwords and creating a new email, they managed to access my accounts again. The other day, while I was using my computer, I saw a strange pop-up that looked suspicious, possibly something used for exploitation. I didn't recognize the applications listed on it, and it disappeared before I could investigate further. Can anyone guide me through the steps to secure my accounts and my computer?

The best step is to do a fresh install of your operating system. If you’re using Windows, you can find tutorials online on how to reinstall it from settings. Thanks for the advice! Is there any specific software I can use to check for issues on my system? I currently use McAfee, but I'm not sure if it's enough.
