I recently switched to a new phone and discovered that my Microsoft Authenticator app wasn't backing up to iCloud. No, you don't necessarily have to create a Microsoft email to back up with iCloud. You should be able to use a third-party email, like Google or Yahoo, for verification. When you register that email, Microsoft will send you a one-time passcode to confirm it's yours. If you're stuck, check if there's a specific option in the app for adding backup emails instead of just the main Microsoft login page.
