Is My One-Year-Old OEM Drive Really Failing?

I have a Thinkpad P16v Gen 1 with two NVME SSDs: a stock Samsung drive and an upgraded SK Hynix 500GB. I wiped both drives and installed Windows 11 on the SK Hynix because I suspected it might be problematic. However, after running some diagnostics, I found that the Samsung drive appears to have a lot of bad blocks, while Lenovo Diagnostics says there's no issue at all. SMART seems to be offline for the Samsung drive, and I'm confused about whether I should replace it or file a warranty claim, especially since Windows doesn't seem to have any issues using it as a secondary drive. Any advice?

It sounds like your OEM drive might actually be bad. It’s not uncommon for drives to show discrepancies in diagnostic tools, but if it’s showing lots of bad blocks, replacing it might be the safest route.
