What’s a good app for rating books, movies, and games?

Hey everyone! I'm looking for a solid app to help me rate various things in my life right now, like books, movies, and games. I really want to avoid websites for this and would prefer something that isn't strictly a mobile app—maybe a program I could use on my PC will work better. Ideally, I want it to function like modern rating lists, but I'd also love the ability to add my own information and pictures. Any suggestions?
