You could look into getting an HDMI breakout audio box. This would allow you to extract the audio signal separately, so you can send it to an amplifier or direct connection for your mom's earphones without interrupting the TV sound.
If your TV has a line out or headphone jack, you can split that output into two connections. One can go to some speakers (even basic ones) and the other can connect to her earphones, whether they're wired or through a Bluetooth transmitter. Just keep in mind that some TVs may turn off the internal speakers when you plug into the headphone jack, but you might find options in the settings that allow both to work.
